HOW TO APPLY

ELIGIBILITY REQUIREMENTS

Candidates must be eligible for licensure with the BC College of Pharmacy and have completed an accredited year 1 pharmacy practice residency program (ACPR) and/or an accredited Clinical Masters (in Pharmacy) degree, or equivalent combination of education and experience.  Additional clinical experience is preferred.

 

Interested Applicants should submit the following via email to avena.guppy@vch.ca:

  1. Curriculum vitae

  2. Letter of intent (maximum 500 words). Please indicate which LMPS programs you wish to apply to; you may apply to more than one program with a single application package

  3. Copy of residency certificate or verification of successful completion supplied by the program (signed letter or email from the program coordinator)

  4. Three letters from professional references

  5. Application fee: $120 payable as a cheque, money order, or bank draft payable to “Provincial Health Services Authority”.  Mail cheque to: Attention Avena Guppy, Administrative Assistant – Lower Mainland Pharmacy Services, 2733 Heather Street, Heather Pavilion, Level D, Vancouver, BC, V5Z 1M9

FEES

There will $120 application fee for the 2022-2023 academic year.
